Chronic Obstructive Pulmonary Disease近日刊登了一篇关于慢性阻塞性肺疾病患者的家庭肺康复治疗:一项随机临床试验《Home-based pulmonary rehabilitation in patients with chronic obstructive pulmonary disease: a randomized clinical trial》。

背景:肺康复(PR)是一项针对慢性阻塞性肺疾病(COPD)患者的多学科护理计划,旨在提高COPD患者的活动能力和生活质量,并维持COPD患者的临床稳定性。然而,尽管很多患者对自己目前的状况感到不适,并非所有患者都能接受肺康复。


目的:本研究旨在评估家庭肺康复计划对活动能力、生活质量、呼吸肌力量和活动耐力的影响。


方法与措施:根据慢性阻塞性肺病全球倡议,符合慢性阻塞性肺病诊断的患者被随机(双盲)分为两组,一组在家进行有氧和肌力训练,称为干预组;另一组只接受呼吸和伸展运动的指导,将其描述为对照组。


我们跟踪随访了以下内容2个月:运动耐力(递增穿梭行走试验和上肢试验)、呼吸肌(力量和耐力试验)和健康相关的生活质量(呼吸道疾病问卷)。


结果干预后,两组的运动耐力和生活质量均无显著变化,然而,与对照组相比,干预组的呼吸耐力有所提高,而对照组在HBPR后呼吸肌承受的负荷有所下降。


结论每两周监督一次的HBPR计划,虽然不足以在体力或生活质量方面提供改善,但在维持COPD患者临床特征的稳定性方面发挥了重要作用。根据每日记录的信息,患者在干预期间没有发生症状恶化。
